How can I request a copy of my medical records?

Please be advised that in order to have access to or obtain copies of your medical/clinical health records, an official request for medical records must be made in writing.  Patients and other qualified persons must complete the form Authorization for Release of Medical Records.

A copy fee may be charged for medical records as per state law but you will be informed of such costs prior to shipping.
